Patient Information: Female, Age 85
Diagnosis: Falls / Back Pain
History: This woman, a long-term skilled nursing facility resident, was referred to therapy by the
nursing staff due to a recent fall in her room, decreased safety awareness, unsafe transfers,
and increased pain in her back and legs. Prior to the fall this woman required supervision to safely get in and out of bed and maximal assistance to walk.
Pre-Therapy Status:
•Hip and Back Pain: Severe (8/10).
•Functional Mobility: Maximal (>75%) assistance for bed mobility and transfers.
•Standing Activity Tolerance: 1 minute with maximal (>75%) assistance.
•Ambulation: 3 to 4 feet with moderate to maximal (50-75%) assistance.
•Wheelchair Mobility: Independent for short distances with assistance for positioning.
Therapy Information:
•Modality: OmniStim® 500 Pro Electrical Stimulation.
•Frequency: 5x per week.
•Protocol Specifics: Interferential current electrical stimulation to the low back for pain.
•Duration: Four weeks.
•Other Therapy Services Provided: Transfer training, balance exercises, gait training, therapeutic exercises, and wheelchair management training.
Outcome:
•Hip and Back Pain: Absent (0/10).
•Functional Mobility: Independent for bed mobility; independent with close supervision for transfers.
•Wheelchair Mobility: Independent throughout the facility.
•Standing Activity Tolerance: Independent for five minutes with close supervision.
•Ambulation: Throughout the facility with guiding contact of a caregiver for steadying assistance.
This woman and her family are very excited about her accomplishments in therapy! Since she is now pain-free in her hip and back, she is able to independently maneuver throughout the facility using her wheelchair and is walking with staff to and from activities and meals.
